Assisting you with navigating through your insurance benefits questions and payment is an important part of what we do. From benefits verification and treatment pre-authorization through treatment and rehabilitation, you will deal directly with us.

What insurance do you accept?

What if Lake Orthopaedics is not on my plan?
If we are not on your plan, you may still be able to see us, but you will need to understand your in-network and out-of-network benefits. Your insurance company can provide these details for you.
What if I don’t have insurance?
Patients who are not covered by health insurance will be required to pay for services as they are rendered. Payment arrangements can be made.
Forms of Payment
We accept cash, check, Master Card, Visa, American Express, and Discover.
Under-age Patients
If the patient is under 18 years, a parent/guardian must accompany the patient. If the parent/guardian cannot be present, a note of permission must be presented with the patient before they can be treated.

Forms
To save time at your appointment, we encourage you to download and complete the appropriate forms before your visit. You may fill in and print from your computer or print out and fill in by hand.
Please hand carry all forms with you to your appointment. Do not mail.
Appointment Check List
Please bring the following to your appointment. If you are unable to bring all of the information listed, please call so that we may reschedule your appointment.
- Driver’s license or photo ID
- Your insurance card(s) along with your co-pay which is due at time of visit.
- Referral if necessary
- Downloadable forms provided in advance
- Prior test results regarding your problem (example: EMG or lab results)
- X-ray on CD including MRI, C.T. scan, etc. along with the reports
- Any other information that may help your physician
For Workers’ Compensation Patients:
- W.C. Card with your MCO information
- C-9 if needed
- Employer information (name & phone number of contact person)
